Option 1:

Please submit the BEAR Grant Evaluation Summary below. Write a brief response for each statement. Email photos and/or videos to info@wblaef.org.

Option 2:

If you prefer, instead of a written evaluation summary, you may attend a WBLAEF Board meeting and give a 3 to 5 minute presentation to the WBLAEF Board summarizing your experience and the impact of this grant. Please call the Foundation Office to schedule your presentation at 651-407-7696 or email info@wblaef.org.
